STEP 1

Undergo Assessment
A nurse will conduct a home visit to assess your eligibility for the PAS program. During this assessment, you will also be informed about the number of weekly care hours you qualify for.

STEP 2

Select Your Caregiver
You have the freedom to choose your caregiver, whether it's your daughter, son, a relative, or a friend. We will come to your home to assist with your enrollment in the program.

STEP 3

Receive Care — They Receive Compensation
You will receive continuous care from a loved one, while they gain peace of mind knowing they are being compensated for the care they provide.

To Get Started With Us

1. INITIATE CONTACT OR REFERRAL: Feel free to reach out to us by dialing 347-701-4567. During a brief 5-10 minute call, we'll address any immediate questions and schedule an in-person meeting.

2. IN-HOME CARE EVALUATION: We'll respond to all your inquiries and pose some of our own to create a Customized Care Plan tailored to your specific requirements.

3. COMMENCEMENT OF CARE: On the first day of care, our Caregiver will accompany us to review the Care Plan together and facilitate introductions.

4. CLIENT VISITS: Depending on your preferences and needs, we'll maintain regular visits or include you in our 24/7 check-in program.
